Informed Consent

Purpose:

The researcher requests your agreement for participation in a study regarding the usage

rate and opinions of your skin care products and routine. The questionnaire is conducted only for academic purposes. By participating in the survey, you allow us to use your feedback to enhance our knowledge of the topic.

Methodology:

You are asked to complete the questionnaire regarding to your lifestyle, observations, preferences, and underlying motivations for using skin care products. Our intent is to accurately describe and classify the group of people who participate in the study.

Types of Questions:

You are inquired to answer questions about your habits, behaviors, and predispositions in relation to your skin care routine. Demographic information, such as age, gender, and ethnicity will also be asked so that we may more adequately understand our target market. The survey will take less than 15 minutes to complete.

Risks:

No risks or discomfort are anticipated from being a part of the study. If you decide not to

complete the questionnaire after you start it, your answers will not be recorded. There are no adverse consequences if you do choose not to answer specific questions, or opt out of the questionnaire.

Participation:

Your participation is voluntary. Your name will stay anonymous, and we value your confidentiality. The information gathered from the study will solely be used for our internal

marketing purposes, and will not be shared with any third parties. We highly appreciate your contribution.
